The document outlines the quadruple burden of disease that South Africa is faced with, furthermore highlights the extent to which the health system struggles with four major health issues: non-communicable diseases (NCDs), communicable diseases (especially HIV and TB), maternal, neonatal and child morbidity and mortality, and deaths due to injury and violence. Health promotion has a significant role to play in reducing the burden of disease to the health system, by addressing the key social, behavioural and structural determinants of health.

South Africa (SA) is one of the 30 high burden tuberculosis (TB) countries contributing 87% of the estimated incident TB cases worldwide, on its own SA accounts for 3% of cases globally. Among these 30 high burden TB countries, the country is among the 14 countries with the highest burden of TB, TB/HIV and multi-drug resistant TB (MDR-TB). The country’s TB epidemic is driven by a number of factors including low socio-economic status and a high HIV coinfection burden.

. The goal of the national antenatal sentinel HIV prevalence survey is to produce robust scientific evidence at national, provincial and district level in order to: Determine the HIV sero-prevalence among first time antenatal clinic attendees, seen as a particularly suitable “sentinel” group to represent most closely the HIV prevalence of the general adult sexually active part of the population and to provide baseline empirical data for national, provincial and district level estimates and extrapolation of HIV infections for future projections of the epidemic.

This report contains a snapshot of the use and impact of antibiotics in the environment. Surveillance within the water-plant-food interface remains limited in South Africa currently, although research is being done in various aspects e.g., identification and characterisation of multidrug-resistant potential foodborne pathogens from fresh produce and irrigation water. Increased capacity and integration into other structures will need to be investigated to improve the surveillance capacity in this area.
