The Adult Primary Care Guide (APC) is a clinical decision-making tool for health care practitioners working at primary health care level in the public health sector in South Africa. APC is designed to support integrated clinical consultation by prompting navigation between the three sections: general health, presenting symptoms, and long-term conditions.It enhances quality person-centred care by upholding a set of key values:
• Acknowledgement of each patient’s uniqueness and multiple roles within a family and community
• Respect for a patient’s concerns and choices
• The development of a trusting relationship with a patient
• Communication with a patient that is effective, courteous and empathic
• The delivery of follow-up care, especially for patients with chronic conditions
• Linking the patient to community-based resources and support
• Ensuring continuity of care, where possible
