This document sets out SOPs for the DHMIS policy, for use by health facilities, which are the first level of interaction between community members and health services. These SOPs seek to achieve standardization in data collection, capturing, collation, storage, analysis and transmission to other levels of the health system.These SOPs present basic and practical steps to be followed by health care providers and health information management personnel to ensure that data is appropriately handled and used to improve service delivery at local level, prior to submission to the next level of the health system, within the specified time frames.
